You may bring the forms in at the time of your visit or submit them prior to your visit. These include the diagnosis form, consent form and intake form.
*Please note that the forms are needed in order to be seen by us. It is your responsibility to confirm that the forms have been received and that you're sending them prior to your visit.

Visit Dispensary of Choice
Head on over to the dispensary of your choice to complete the registration process for the state.
Ensure to bring your ID, proof of address and $100 cash.
